Anesthesia services are considered completed when the patient may be safely left under the observation of trained personnel and the doctor may safely leave the room to attend to other patients or duties. Scaling and debridement in the presence of inflammation or mucositis of a single implant, including cleaning of the implant surfaces, without flap entry and closure. (When submitted with prophy, considered inclusive of prophy; no separate benefit for 6081 or when submitted alone or in multiples, allow to pay as prophy, but subject to prophy limitation.)

Incomplete endodontic therapy - inoperable or fractured tooth.

Endodontic therapy - molar tooth (excluding final restoration). The fee for palliative treatment is Disallowed when done In Conjunction With root canal therapy by the same dentist/dental office on the same date of service. Palliative treatment is payable on a separate date of service for relief of pain. Incompletely filled root canals are not payable, and the fee for the endodontic therapy is Disallowed. Post removal is not included in this procedure.
